Our Clinical Pathway
Our Clinical Pathway
At Inmind Healthcare, we are dedicated to delivering high-quality, patient-centred care through a structured and evidence-based clinical pathway. Our approach ensures that individuals experiencing complex mental health conditions receive the right level of support, tailored interventions, and a clear route to recovery. The clinical pathway is designed to guide patients through each stage of their treatment journey, from initial assessment and stabilisation to rehabilitation, reintegration, and long-term recovery.

Introduction to Our Clinical Pathway
Our clinical pathway is a structured framework that defines the stages of care a patient will go through, ensuring consistency, safety, and effectiveness. The pathway is underpinned by principles of trauma-informed care, the least restrictive practice, and multidisciplinary collaboration.
We recognise that each patient’s journey is unique, and while the pathway provides a framework, care plans are highly individualised. Our pathway integrates various treatment modalities, risk management strategies, and therapeutic interventions to support recovery.
The Key Stages of Our Clinical Pathway
The pathway at Inmind Healthcare follows distinct but flexible phases to ensure that each patient receives the appropriate level of care at the right time.
A. Assessment and Admission
Upon referral, our multidisciplinary team (MDT) conducts a comprehensive pre-admission assessment to determine the suitability of care. This includes:
- A full diagnostic assessment, including mental health history, risk assessment, and physical health review.
- Liaison with previous care providers, families, and commissioners.
- An evaluation of individual needs, strengths, and goals.
Patients are either admitted informally or under the Mental Health Act, depending on their needs. The transition into care is carefully managed to ensure a smooth and supportive experience.
B. Stabilisation and Early Treatment
The stabilisation phase focuses on:
- Risk management: Our clinical team implements structured risk assessments such as HoNOS, HCR-20, START, and the Recovery Star.
- Medication management: Our medical team reviews existing prescriptions and ensures patients receive the most effective medication with regular monitoring.
- Psychological support: Patients begin structured psychological interventions, including Cognitive Behavioural Therapy (CBT), Dialectical Behavioural Therapy (DBT), and trauma-focused therapy.
- Occupational therapy: Early interventions help patients establish a daily routine, focusing on independent living skills and engagement in meaningful activities.
C. Skills Acquisition and Therapeutic Interventions
This phase is critical in helping patients develop coping mechanisms, social skills, and greater independence. Core elements include:
- Individual and group therapy sessions: Evidence-based therapies such as Schema Therapy, Mentalisation-Based Treatment (MBT), and psychoeducation.
- Substance misuse support: For patients with co-occurring conditions, targeted interventions support addiction recovery.
- Social inclusion and family interventions: We involve families in the recovery process through structured family therapy and support sessions.
- Daily living and vocational training: Through our occupational therapy programme, patients develop skills to support future independence.
D. Community Integration and Transition Planning
As patients progress in their recovery, the focus shifts to preparing them for life beyond inpatient care. Our rehabilitation pathway ensures a structured yet gradual transition:
- Step-down services: Patients may move from high-support wards to semi-independent living arrangements within our facilities.
- Real-world exposure: Gradual community engagement, supported leave, and skill-building activities ensure patients develop confidence in navigating daily life.
- Relapse prevention planning: Comprehensive aftercare plans are developed, ensuring patients have the necessary support in place upon discharge.
E. Discharge and Aftercare Support
The final stage of our pathway focuses on sustained recovery:
- Aftercare services: Patients receive continued access to therapy, peer support groups, and follow-up care from community teams.
- Family and social support: Ongoing involvement of families and carers to reinforce stability.
- Employment and education support: Where appropriate, patients are guided towards employment, volunteer work, or further education to enhance their sense of purpose and independence.
Our Therapeutic Approach
The clinical pathway at Inmind Healthcare is underpinned by a holistic and person-centred approach. We integrate a range of therapeutic models to meet individual needs:
A. Psychology-Led Interventions
Our psychology team delivers structured therapeutic interventions, including:
- Cognitive Behavioural Therapy (CBT) – Helps patients challenge unhelpful thoughts and develop healthier coping mechanisms.
- Dialectical Behavioural Therapy (DBT) – Supports individuals in managing emotional distress, impulsivity, and interpersonal difficulties.
- Trauma-Focused Therapy – Addresses past traumatic experiences and promotes healing and resilience.
- Schema-Focused Therapy – Explores deep-rooted patterns of behaviour and works to change negative life scripts.
B. Occupational Therapy and Skill Development
Our occupational therapy programme enables patients to build confidence and independence by focusing on:
- Daily living skills (cooking, cleaning, budgeting, self-care)
- Vocational skills (education, job readiness, volunteering)
- Leisure and social engagement (creative therapies, group activities, peer support)
C. Medication Management
Our medical team ensures that medication is optimised to support mental health stability, reduce side effects, and enhance overall well-being. Patients receive regular reviews and education about their medications to promote adherence.
D. Physical and Holistic Health
Recognising the strong link between physical and mental well-being, we offer:
- Physical health monitoring – Regular assessments, dietary support, and structured exercise programmes.
- Mindfulness and alternative therapies – Yoga, meditation, and art therapy to support emotional regulation.
- Gym and outdoor activities – Access to structured exercise sessions to promote overall health and well-being.
